For this reason, we require a completed intake packet before services can be performed. The packet includes forms about you and your health.  . The intake questionnaire should be completed honestly, and not in a rush. This information is vital to a thorough naturopathic treatment plan; furthermore, this information is the gateway that leads the practitioner inside who the client is and their specific needs. The intake questionnaire may take several minutes to complete, so please be patient and attend to detail.

 The naturopathic path in itself is a lifestyle decision that depends more on you and your dedication, rather than on your practitioner. You are the star and only you can play a major role in your overall improvement.
